HEALTH bosses in Lanarkshire have welcomed a 60% drop in delayed discharges from hospital compared to this time last year.

The drop is thanks to a partnership between health and social care staff introducing a programme called Planned Date of Discharge.

Staff work with patients’ families and medical professionals in order to assess the needs of those leaving hospital so that they can return home safely. The drop has created an increase of 70 beds since May.

Val de Souza, Chief Officer of South Lanarkshire Health and Social Care Partnership, said: “What we have introduced now is an improved system of working, with smoother, more seamless, links between NHS staff, the hospital-based social care workers and the community team.
